Q:  What if my surgeon determines that I need to go to a transitional care facility?

A:  If your surgeon thinks you need to go to an inpatient rehab (aka, a transitional or sub-acute care) facility, the decision of where you go will depend on your needs. It may also depend on which facility has a bed available. Your insurance company may influence the decision as well.

Find out about your options in advance and discuss them with your clinical care coordinator. Think about what is the best location—near your home, near the hospital or near your family members. While you may not get your first choice, it’s a good idea to be prepared with options so you can make your preferences known.
